Please read these instructions before attempting to use the ULLR public forms.
These forms and documents are provided for downloading to
your PC. Once they are saved to your PC, you can then fill them
out and print them. They are not intended to open within your browser and fill out online.
Attempting to fill out the forms online may cause you to lose your
inputs, and make it difficult to save a copy on your PC.
If an "Enter Password" dialog box appears at any time, just click the "cancel" button and continue.
Some
of the Excel spreadsheets contain macros which are necessary for the
form's calculations. If prompted whether to allow the macros to run in
Excel click "Yes", or if you get a Security Warning stating that "Macros have been disabled" in Excel you must change the security option to "Enable this content".
To
download a form using Microsoft Windows and an Internet standards
compatible browser (Edge, Internet Explorer, Chrome, Firefox, Safari...),
right-click on the desired hotlinked file format
(PDF, DOC, XLS) and select Save (Target) As; in some browsers this can
also be done from the main menu bar using File/Save As... Your PC
should then prompt you to pick a Save location on your local hard
drive. Once saved, you can open the file with the appropriate
program (Acrobat Reader, Word, Excel), fill it out, and print the form.
To
print the form online, just left-click (Mac ctrl-click) on the
hotlinked file format. Most browsers should support on-screen
viewing of the files. When the form appears on your screen, just
print the page in your browser. (this may not work for all
browsers; some may initiate a download of the form)
